Includes all non-OHIP bills covered by the Block Fee Program for a period of one year. The Family Block Fee applies to the patient, their spouse or partner, and dependents under 21 years of age.  All members must reside at the same address. Only individuals who are patients of MyFamilyMD are covered by the Family Block Fee.
